We aim to make referrals straightforward and responsive. We know that when you’re making a referral, time and clarity matter. Whether planned or urgent, we respond promptly, review thoroughly, and communicate clearly every step of the way.
We are currently accepting referrals for:

How to make
a referral.
We keep the referral process simple and responsive.
Step 1, Initial contact
Email us a summary of the individual’s needs and support requirements.
Step 2, Information sharing
We’ll request relevant documents, which may include:

Needs assessment

Risk assessments
-
Care plan or support history
-
Social worker or case manager contact details
Step 3, Next steps
We’ll review suitability and respond within 24 to 48 hours. Where appropriate, we’ll arrange an assessment or placement planning meeting promptly.
