Supported Living Plus

Transitional Supported Living for Young Adults (17–25)

Our Supported Living Plus service is designed for young adults and working-age adults who are ready to move towards greater independence but need the right support and structure to succeed.

With adapted housing, small-scale living (typically 2–3 tenancies per site), and a structured wraparound support model, this is a progression-focused service, not long-term dependency. We walk alongside individuals as they build confidence, skills and community connections, moving forward at a pace that works for them.

What we offer:

  • Structured supported living with clear progression planning

  • Small, carefully matched shared settings

  • Adapted homes to support sensory and safety needs

  • Wraparound support including:

  • Positive Behaviour Support-informed practice

  • Life skills development

  • Mental health and emotional wellbeing support

  • Support with education, employment and community access

  •  Clear independence goals and move-on planning

Who it’s for:

  • Individuals transitioning into adulthood

  • People stepping down from inpatient or unstable placements

  • Adults with autism, learning disabilities or related needs

  • Those who require a structured pathway towards greater independence

  •  Individuals who benefit from smaller, consistent environments

How we work:

  • In partnership with commissioners, health professionals and families

  • Focused on safe transitions and continuity of care

  • Flexible, responsive and always person-centred

We are building services that support progress, strengthen independence and create realistic pathways into community living.
