Supported Living Plus
Transitional Supported Living for Young Adults (17–25)
Our Supported Living Plus service is designed for young adults and working-age adults who are ready to move towards greater independence but need the right support and structure to succeed.
With adapted housing, small-scale living (typically 2–3 tenancies per site), and a structured wraparound support model, this is a progression-focused service, not long-term dependency. We walk alongside individuals as they build confidence, skills and community connections, moving forward at a pace that works for them.
What we offer:

Structured supported living with clear progression planning
-
Small, carefully matched shared settings
-
Adapted homes to support sensory and safety needs
-
Wraparound support including:

Who it’s for:
-

-
People stepping down from inpatient or unstable placements
